Paying by Check or Money Order

If you wish to pay by check or money order, then download the  order form.

The form contains all the information you will need to complete your order, including our mailing address and a list of our products and prices. It is an Adobe PDF file. You will need Adobe Acrobat Reader to view and print it (free).
